Mobile Forms Article

Mobile Forms can be manually sent to the desired recipient(s) by email or as a mobile-friendly SMS text message from within the claim record in the ClickClaims application. This feature provides the recipient with seamless access to the form via a hyperlink located within the email/text message allowing them to populate and submit it online from any device.  Once submitted, the completed form, along with any images and videos included with it, are automatically imported into the claim.  This article provides a comprehensive review of this feature.

 

Sending the Online Form

The mobile forms are available as an email and an SMS text message (requires that the “SMS Text Messaging” feature is enabled in ClickClaims) which can then be automatically or manually sent to the recipient for online completion and submission.

Email:

  1. An authorized user can manually trigger a mobile form via email by selecting the desired form template from the  button which is available on any claim tab within the ClickClaims application.

  1. Note: Your company may have one or more automated processes in place which will automatically trigger a specific mobile form via email to the recipient(s) based on predefined conditions.   If you are unsure if this applies to your company’s workflow, please contact your manager for details.
  2. Note: The mobile forms listed under the  button can vary for your company.  
  3. The system email templates used to generate each form can be managed by a site administrator from the “Manage System Emails” page accessed under the “Tools” menu bar button.  Templates used for the base mobile form emails include:
    1. Hurricane Triage Mobile Form: GE11 – Hurricane Triage Form
    2. Water Damage Mobile Form:  GE12 – Water Damage Form 
    3. Lightning Damage Mobile Form: GE13 – Lightning Damage Form
    4. Wind Damage Mobile Form: GE14 – Wind Damage Form

2. When the email is generated either automatically or manually, an activity note is created on the claim’s “Notes” tab.

  1. If automatically generated, the activity note is created under the “System” note type and “Email” sub-type.   If manually, the activity note is created under the “Company User” note type and “Email” sub-type.

b. A copy of the manually generated email is also saved as a PDF document under the “System Documents” file type of the claim’s “Documents” tab.

SMS Text:

  1. If your company has incorporated SMS Text Messaging into your workflow, a text message can be manually triggered by an authorized user by selecting the desired form template from the  button which is available on any claim tab in the ClickClaims application.

5. IMPORTANT:  The recipient must have an opted-in phone number in order to receive the outbound SMS Text message from ClickClaims.  

2. When the text message is generated either automatically or manually, an activity note is created on the claim’s “Notes” tab under the “System” note type and “Text Message” sub-type.

Populating the Form

  1. Once the email/text message is received by the recipient, they can access the mobile form via the hyperlink that’s contained in the email/text.

2. Upon clicking the hyperlink, their device’s default browser directs them into the form where they can begin to populate it.

3. Once they complete the form, they must click the  button to submit the form or the  button to modify prior pages.  

 

4. Once the  button is selected and the process is successful, a confirmation message displays in their browser.  They’ll also receive a confirmation email.

5. Approximately every 15 minutes a scheduled task runs to identify newly submitted forms.  When a new form is identified, a PDF copy of the completed form, along with any images/videos attached to the form, are automatically uploaded to the applicable claim in ClickClaims and saved under the “Form Submissions” file type and “Form Submissions” sub-type of the claim’s “Documents” tab.

6. When applicable, please allow approximately 15 minutes after form submission for information to update the related claim field(s)/flag(s) within the ClickClaims application.  An activity note may be created on the claim’s “Notes” tab under the “System” note type and “EDI” note sub-type recording these changes.  

  1. If you are unsure what mobile form information, if any, will synch back to the claim, please contact your manager.
